Use this Coach Feedback form to assess the performance of a TWOA referee. This form should be used to report outstanding, as well as, poor referee performance.

Your objectivity is essential – please consider the referee’s performance without regard to the outcome of the event.

Your feedback will not be shared with the official unless you specifically state so on the feedback form. Also, an email will be sent to you to confirm your feedback submittal.
